In Part I of the form, please indicate only new ministry areas you would like to serve in 2012.  (In Part II of this form, you will list ministries you are currently serving in 2011 and will continue to serve in 2012.)  Part I is divided into 14 sections, each representing an area of ministry (i.e., music, youth ministry, missions).  Please check the boxes of the ministries you will commit to serving in 2012.  Each adult and youth in your household should complete a separate form.
